This is a request under the Illinois Freedom of Information Act. I request copies of any datasets,
databases, spreadsheets, or logs maintained by the City regarding all citizen complaints, internal
investigations, disciplinary actions, and appeals of discipline involving sworn officers of the
Evanston Police Department (EPD) any rank. Specifically, I request as much of the following
information as is maintained by the EPD in a spreadsheet or log format, dating back as far as is
maintained in such a format:


● For every internal investigation, whether stemming from a citizen complaint or internal
complaint, please provide:
○ Case number
○ Date of complaint
○ Date of complaint incident
○ Location of complaint incident
○ Location type of complaint incident
○ Nature/type of complaint
○ Whether a citizen or internal complaint


○ For every Department employee, of any rank, accused in every complaint, please
provide:
■ First name
■ Middle name or initial
■ Last name
■ Rank
■ Any badge/star number that has been assigned to them
■ Employee number
■ Year of birth
■ Gender
■ Race
■ Appointment/hire date

○ For every allegation against every Department employee accused in every
complaint, please provide:
■ Allegation/charge
■ IAD finding (i.e., sustained, not sustained, exonerated, etc.)
■ Date of finding
○ For every complainant, please provide:
■ Race
■ Gender
■ Year of birth
■ If the complainant is a Department employee, please also provide:
● First name
● Middle name or initial
● Last name
● Rank
● Any badge/star number that has been assigned to them
● Employee number
● Year of birth
● Gender
● Race
● Appointment/hire date

○ For every witness to the complaint incident, please provide:
■ Race
■ Gender
■ Year of birth
■ If the complainant is a Department employee, please also provide:
● First name
● Middle name or initial
● Last name
● Rank
● Any badge/star number that has been assigned to them
● Employee number
● Year of birth
● Gender
● Race
● Appointment/hire date

○ For every investigator assigned to each complaint/investigation, please provide:
■ First name
■ Middle name or initial

■ Any badge/star number that has been assigned to them

■ Appointment/hire date
● For every disciplinary action, please provide:
○ IAD case number
○ Any other case number
○ Disciplinary action being taken
○ Date of filing
○ Whether the action was appealed
■ If so, date of appeal
■ If so, final disposition of appeal
■ If so, date of final disposition

In the event the City redacts or withholds any documents under IL FOIA, please provide the
legal reasoning behind every redaction or withholding.
