Under the Illinois Freedom of Information Act I am kindly requesting actual copies or histories of all water and sewer bills issued for the previous twelve months for any and all accounts or meters which service the following facilities, noting that service address may vary:

Evanston Hospital

2650 Ridge Ave, Evanston, IL

 

The responsive documents should include customer name, service address, and itemized water/sewer consumption and billed amounts by bill cycle. Additionally, please provide copies of any applicable rate schedules where available.

 

Please send this information in PDF format or similar via e-mail.

 

Should the payment of any fees be necessary in order to proceed, please provide a cost estimate prior to providing any records.

 

In the event that any portion of this request is to be denied, please provide citation of each specific exemption which justifies the refusal to release the information and advise as to the appeal procedures available.
